Medical Records Release Form
Please download and print a copy of a medical release form so that we may obtain and/or share information with the appropriate providers in your personal medical network.
Consent for a Minor
If you have an upcoming visit for your minor child, please download and complete the consent for treatment form prior to arrival.
General Wound Care Instructions
Keep the wound dry for the first 24 hours. After that, clean it daily with soap and water, apply Vaseline or Aquaphor, and cover with a bandage. Download your instruction copy here.
Medical Records Release Form
If you need to request or transfer your medical records, please complete the Medical Records Release Form. This form allows you to authorize the release of your medical information to another healthcare provider, facility, or authorized individual.

Whether you visit the dermatologist regularly or are a new patient seeking expert advice, we are here to help with all your skin, hair, and nail needs. When scheduled for an appointment at one of our locations, we ask that you please bring a photo ID and your insurance card with you to the visit.
